hi I'm dr. Guymon one of the things
that I'm commonly asked in the exams
that I do is What does it feel like to
get braces put on? Does it hurt to get braces put on? How long does
it take to get braces put on? and What's it like getting braces put on?
00:12
and so I thought it would be
00:13
kind of cool that show a little video of
00:15
us putting on braces on one of our
00:17
patients and so that's what we're going
00:19
to go through right now is step by step
00:21
what we do or there's no magic to it and
00:25
so I wanted to show you the video so
00:27
that you felt comfortable of seeing what
00:29
you were in for and what that whole
00:31
experience is going to be like okay
00:33
we're going to walk you through putting
00:35
on braces and I'm going to use this
00:37
patient that we're going to call her
00:38
Sarah we're going to put on Sarah's
00:41
braces right now and we start off by
00:42
basically putting on a little cheek
00:44
retractor and this cheek retractor holds
00:47
the lips and cheeks away from the teeth
00:49
which you'll see then we also use a
00:51
little device that holds the tongue away
00:52
from the teeth now the reason we want
00:54
these guys in there is we want to keep
00:56
the teeth as dry as possible once we
00:59
start preparing the teeth and working on
01:00
them we don't want the lips the saliva
01:03
or the tongue to interfere with what
01:05
we're doing and here you see that we're
01:07
painting each individual tooth with a
01:09
little cotton applicator and we're
01:12
basically sterilizing the tooth surface
01:13
and preparing it for the glue this glue
01:16
is a special kind of glue that sticks to
01:19
tooth surfaces only after they've been
01:20
prepared with this special material once
01:23
we put the sterilization agent on we
01:25
rinse it and then we dry it off we want
01:28
the teeth to be as dry as possible and
01:30
then here you'll see me come in and put
01:32
on each of the braces one at a time now
01:35
the braces are the bracket we call them
01:37
brackets each have a little bit of glue
01:41
on them and we put them on the tooth
01:43
just like you'd put a sticker on
01:44
something so you put them on just like a
01:46
sticker but the glue is not yet set or
01:48
dried so it allows us to work with the
01:50
braces so at this point we've had we
01:52
have all the braces on the teeth one of
01:55
the things you might notice at this
01:56
stage also is that each of the teeth has
01:59
a very specific bracket for it and so as
02:02
a matter of fact we use six different
02:05
types of brackets on the upper arch and
02:07
five different types of brackets on the
02:10
lower arch
02:11
and they're a little bit different in
02:13
shape and dimension some of them have
02:15
hooks for rubber bands to where you can
02:18
see those hooks on the brackets
02:20
themselves and and so they're all very
02:24
specific for the individual tooth
02:25
they're they're moving here you can see
02:29
that we're actually trying to get the
02:31
angles of the brackets right so we're
02:34
positioning them with a with what we
02:37
call a scaler and now we're going in and
02:39
we're actually measuring the height of
02:41
each bracket now each bracket along with
02:43
each tooth out in a specific bracket
02:45
each mm for ideal bracket placement and
02:49
so I'm using this measuring device and
02:51
holding it at different angles to get
02:53
the brackets right at the right
02:55
measurements on each specific tooth a
02:57
couple things I want to talk about with
02:59
the specific brackets is that within
03:02
those brackets they actually have a what
03:05
we call a prescription built into them
03:07
that prescription once we put a wire in
03:10
that prescription dictates the
03:12
angulation and the torque or inclination
03:16
of each individual tooth as well as the
03:18
height of each individual tooth within
03:20
the mouth so one of the things that's
03:22
important to me is when I go in and
03:24
start putting on the braces I can
03:26
already envision the teeth and their
03:30
final position so I kind of begin with
03:33
the end in mind I can already see the
03:34
final results I can see the final smile
03:36
before I even put on the braces
03:41
now here you see the special light it's
03:45
a basically a visible within the visible
03:48
spectrum so it's not a laser or anything
03:50
like that there's no heat that's
03:51
generated from it it's just a very
03:53
intense light that acts as a catalyst to
03:57
make the glue harden and each bracket
03:59
needs to be cured approximately 10 15
04:02
seconds and we move on one bracket at a
04:04
time and you can see as we finish moving
04:07
the light across the glue that bracket
04:10
is then set and it becomes a permanent
04:13
permanent fixture on the teeth until we
04:15
decide to take that bracket off and see
04:24
really at this point there's not really
04:27
any pain that goes into putting on the
04:28
braces like I said it's just like
04:30
putting stickers on the teeth doesn't
04:33
feel real normal at first she has a hard
04:35
time getting your lips over her teeth
04:37
over her braces everything feels real
04:39
bulky and thick now once we've got all
04:42
the braces on we move to the next step
04:46
which is putting the wires on to the
04:50
braces themselves okay at this point
04:54
sarah has all of her braces on we've
04:57
used the light to make the glue hard and
04:59
now she's ready for the next step which
05:01
is to put her wires in now these wires
05:04
are made of well we call them copper
05:07
night tie wires they've got it basically
05:10
this material was developed by NASA
05:12
quite a while ago and it is a
05:14
temperature sensitive wire that is also
05:18
a shape memory wire now what that means
05:20
is that this wire is very flexible but
05:25
when it hits body temperature when it
05:27
starts to warm up the wire always
05:30
returns to its original straight shape
05:31
so it's not like your standard wire that
05:33
you'd find in you know building
05:35
materials or or what have you it's a
05:37
shape memory wire and so what that means
05:40
is as we attach the wire to each
05:43
individual bracket and you'll see that
05:45
we're putting on her colored ties right
05:46
now these colored ties although they're
05:48
kind of cute and pretty really their
05:50
only purpose is to hold the wire to the
05:52
braces now you'll see this wire
05:55
to get distorted as we go into the more
05:57
crooked teeth and this wire what like I
06:01
said once it hits body temperature it
06:04
actually starts to return to its
06:06
original straight shape so as that wire
06:08
over time straightens out the teeth that
06:11
you can see are crooked or the wire that
06:13
you can see is crooked become straight
06:15
and as the wire becomes straight the
06:17
teeth becomes straight and so really the
06:20
braces are just a handle on the tooth
06:22
the ligature ties those rubber colored
06:26
rubber rubber ties are nothing more than
06:28
holding the wire to the handle and the
06:31
wire is what puts the pressure on the
06:33
teeth and causes the teeth to move now
06:35
at this point she feels a little bit of
06:37
pressure from the wire but again it's a
06:40
very light flexible wire we start off
06:41
with real light flexible wires and work
06:43
up into heavier thicker rectangular
06:45
wires some so the initial wire is very
06:48
light forces but after about 3 to 4
06:51
hours her teeth will start to feel
06:53
tender she won't want to be chewing on
06:57
anything hard so basically her diets
07:00
going to consist for the first three
07:01
days of swallowing type of foods or
07:05
softer foods
07:10
okay so there you go you just saw some
07:13
give you a little clip of what it was
07:15
like getting the braces off now
07:17
obviously that was a little short
07:19
version of the reality of it from
07:21
starting to finish it probably takes us
07:23
about an hour to put on braces and get
07:27
everything done from start to finish so
07:29
it's it's a little longer than what you
07:32
saw there but we're not working on the
07:34
patient the whole time so you're not
07:35
sitting there with your mouth open and
07:37
your cheek stretched out for the whole
07:38
hour but but it does take about an hour
07:40
to get the whole thing done now
07:43
one of the things you notice that we
07:45
don't have needles there's no needles
07:47
there's no poking there's no blood
07:49
there's no crying or or anything like
07:52
that it's a pretty relaxed deal we
07:54
talked through the whole thing now
07:55
there's some points that you can't talk
07:57
because we've got your cheek cheek
07:58
straight out stretched out and your your
08:00
tongue kind of held back but really the
08:04
whole time you're just you should be
08:06
pretty relaxed about this whole deal I
08:07
don't know we put Paige put braces on
08:10
patients as young as six or seven and
08:12
you know really it's not a hard thing to
08:16
go through now I will say that the next
08:18
couple of days after braces
08:20
that's a little more challenging that's
08:21
when your teeth are going to be sore but
08:23
in our office really there's we don't
08:25
there's not a lot of pain and we'll
08:27
you're at the office most of your
08:29
soreness and tooth soreness is more of a
08:32
dull soreness rather than a sharp pain
08:35
and it usually happens a day later you
08:38
know maybe at the earliest four hours
08:40
after after an appointment but during
08:42
the appointment itself you can see
08:43
pretty relaxed pretty easy and again
08:46
shouldn't be a lot of worries about
08:48
what's going to happen here in the
08:49
office glad you watched it hope you got
08:52
something out of it hope watching that
08:55
video set jetties we look forward to
08:57
putting on your braces should be a great
08:59
experience for you look forward to doing
09:00
it for you
